I perioden 2014-2023 har Gode pasientforløp gjennomført regionale læringsnettverk over hele landet, der forbedringsteam fra kommuner og sykehus har samarbeidet om å forbedre overgangene i helse- og omsorgstjenestene. Vi er nå i en ny fase, og KS vil fortsette å støtte kommunene og sykehusenes arbeid med Gode pasientforløp. Dette vil skje gjennom veiledning i planlegging, implementering og gjennomføring av metodikken fra læringsnettverkene. Arbeidet bygger på Helhetlig pasientforløp i hjemmet (HPH) – et rammeverk med strukturert oppfølging ved hjelp av sjekklister. Arbeidet skal også resultere i at brukeren opplever å være likeverdig i utarbeidelse av sitt pasientforløp. Derfor er «hva er viktig for deg?»-spørsmålet så viktig.
