Free β https://pilotdebrief.com/pattern On a clear May afternoon in 2019, two float-equipped de Havilland sightseeing planes converged over the waters northeast of Ketchikan, Alaska β neither pilot saw the other in time. A DHC-2 Beaver operated by Mountain Air Service and a DHC-3 Otter operated by Taquan Air were both returning from on-demand Part 135 sightseeing flights in visual meteorological conditions. The airspace they shared had multiple float operators working the same scenic corridors, with no required separation service and no air traffic control assigning altitudes or routes. About 8 miles from Ketchikan, the two airplanes collided in cruise. The Beaver was destroyed; its pilot and all four passengers were killed. The Otter was substantially damaged but its pilot managed to get the aircraft down β one passenger died, nine were seriously injured, and the pilot walked away with minor injuries. This is the see-and-avoid problem in its starkest form. Two qualified pilots, good weather, unlimited visibility β and a closing geometry that human eyes were never well-suited to detect. The Ketchikan operating environment compounded the risk: high-density tour traffic, predictable scenic routes, and a regulatory framework that left collision avoidance entirely to the cockpit. The lesson reaches well beyond Alaska.
