How do you get rid of bad breath for good? An orthodontist says it usually starts with bacteria on your tongue, not what you ate. Orthodontist Dr. Ben Winters explains where bad breath (halitosis) really comes from and why it's so hard to notice in yourself. He says about one in three people have bad breath, and that eight or nine times out of ten it comes from the mouth rather than from food. Your nose gets used to smells that are always around you, so cupping a hand over your mouth doesn't work; in tests, people worried about their breath rated it correctly only about half the time. A rough home check: lick your wrist, let it dry for about 10 seconds and smell it, though that only samples the tip of the tongue, which is usually the cleanest part. The main source is the rear of the tongue, where its tiny bumps get longer, like a shag carpet, trapping food, mucus and dead cells where saliva can't wash them out. Bacteria that don't like oxygen live there, break down proteins and release volatile sulfur compounds, including the gas behind the rotten-egg smell. A white, yellow or even black coated tongue is a sign. Other sources: food and bacteria between the teeth, gum disease (bad breath that won't go away can be a sign of it), braces brackets, dry mouth during sleep or from mouth breathing and medications, tonsil stones, and less often acid reflux or sinus drainage. Mints only mask the smell, and sugary mints feed the bacteria. He says the research on fixing bad breath is weak; tongue scrapers beat a toothbrush, but not by much, and the coating comes back, so it's a daily job. His routine: scrape as far back as you can, floss daily, choose xylitol mints over sugary ones and alcohol-free mouthwash over alcohol mouthwash. Don't dig at tonsil stones with swabs or fingers. If bad breath lasts two weeks despite all this, see a dentist, then an ENT. • Most bad breath comes from the mouth, not from what you ate. • Bacteria deep on the tongue make volatile sulfur compounds, the rotten-egg smell. • A white, yellow or black coated tongue is a likely source. • Scrape the tongue as far back as you can every day, floss, and pick xylitol mints over sugary ones. • Bad breath that lasts two weeks despite good cleaning is worth a dentist or ENT visit.
