This procedure is designed to replicate natural knee biomechanics while minimizing surgical trauma. Once the socket is prepared, sutures are passed through the joint to facilitate graft insertion. The graft—often an Achilles tendon graft with a bone block—is prepared to match the size and shape of the socket. The PCL TightRope® implant is then attached to the graft, enabling controlled delivery and fixation. The graft is guided into position by pulling on the TightRope® sutures, allowing the bone block to seat securely within the tibial socket. Additional fixation is established as the remaining portion of the graft is passed into the femur and secured, completing the construct. One of the primary advantages of the PCL inlay technique is that it avoids the need for a large posterior incision traditionally required in open PCL surgery. This minimally invasive PCL reconstruction approach reduces soft tissue disruption while improving visualization and accuracy during graft placement. Additionally, this technique helps reduce graft abrasion and allows for better biomechanical alignment, which may lead to improved long-term outcomes.
