Free β https://pilotdebrief.com/pattern On a clear January morning in 2003, a Beechcraft 1900D commuter flight lifted off from Charlotte and pitched uncontrollably nose-up within seconds. Thirty-five seconds later, all 21 people on board were dead. Air Midwest Flight 5481 was a routine US Airways Express run from Charlotte-Douglas to Greenville-Spartanburg, operating Part 121 in daylight VMC. Nineteen passengers and two crewmembers were on board the Beechcraft 1900D as it rolled down runway 18R. Nothing about the morning suggested anything other than a normal departure. The airplane was carrying problems no one in the cockpit could see. It was roughly 580 pounds over maximum takeoff weight, and its center of gravity sat at 45.5 percent of mean aerodynamic chord β well aft of the 40 percent limit. At the same time, recent maintenance had left the elevator control cables rigged so that nose-down elevator travel was limited to about 7 degrees, half of the manufacturer's 14-to-15 degree standard. The two failures collided at rotation. The aft CG drove the nose up, and the captain and first officer simply did not have the elevator authority to push it back down. The airplane stalled at 54 degrees nose high and crashed into a maintenance hangar at the airport. The NTSB later traced the loss of pitch control to the incorrect rigging of the elevator system, compounded by a center of gravity substantially aft of the certified aft limit β with contributing failures spanning Air Midwest's oversight of contract maintenance in Huntington, its weight and balance program, a missed quality-assurance inspection by Raytheon Aerospace, and the FAA's average passenger weight assumptions and oversight of the carrier. The pattern is the one investigators keep finding in commuter and regional accidents: a single hangar-floor error and a paperwork-driven weight assumption can both be invisible to the crew at the controls.
