A 66-year-old patient had undergone laparoscopic staging surgery for endometrial cancer three years earlier, followed by external beam radiotherapy and intracavitary radiotherapy. Subsequently, right-sided grade 3 hydronephrosis developed secondary to distal ureteral stenosis. For this reason, the patient underwent double J stent placement. However, due to the progression of the condition, ureteroneocystostomy was planned for the patient. This video demonstrates a step-by-step laparoscopic ureteroneocystostomy with psoas hitch fixation and Boari flap reconstruction performed for distal ureteral stenosis. The procedure includes meticulous adhesiolysis and ureteral dissection, bladder mobilization, psoas hitch fixation, creation of the Boari flap, ureterovesical anastomosis, and multilayer bladder closure.
