📩 Contact us: [[email protected]] If you've had a hip or knee replacement, you may have been told to take antibiotics before every trip to the dentist. For years, that was standard advice. For most healthy people today, it isn't, and taking antibiotics you don't need comes with risks of its own. In this episode of Talking With Docs, orthopedic surgeons Dr. Brad Weening and Dr. Paul Zalzal answer one of the questions they hear most often: do you need antibiotics before dental work if you've had a joint replacement? They explain why the concern exists, what current guidelines say, who may still need antibiotics, and how to time dental work around joint replacement surgery. Joint replacements are made of metal, plastic or ceramic, and they don't have a blood supply of their own. That's why doctors once worried that bacteria entering your bloodstream during a dental cleaning could settle on the implant and cause an infection. But the evidence has shifted, and so have the recommendations. You'll learn: Why antibiotics before the dentist used to be recommended for life after a joint replacement How bacteria can stick to an implant and hide from your immune system Why dental work puts bacteria into your bloodstream, and why your immune system usually clears it quickly Why scaling and root planing below the gum line release more bacteria than a routine polish Why most healthy people with a joint replacement don't need antibiotics before the dentist What the American Dental Association and orthopedic associations recommend Why bacteria from your mouth are rarely the cause of joint replacement infections Why brushing, flossing and even chewing put bacteria into your bloodstream every day Why antibiotics aren't harmless, from gut microbiome changes to antibiotic resistance and C. diff Why an artificial heart valve is a different story, and still calls for antibiotics Who may still benefit from antibiotics, including people with a previous joint infection, large or complex implants, diabetes, or a weakened immune system How immune-suppressing medications, like prednisone or transplant drugs, can change the decision How long to wait between dental work and joint replacement surgery Why an active dental infection or abscess has to be treated right away, even if it delays surgery One of the most eye-opening points in this conversation is how often bacteria end up in your bloodstream without any dental work at all. If brushing and chewing were enough to infect a joint replacement, these infections would be far more common than they are. It's a big part of why major dental and orthopedic organizations in Canada and the US no longer recommend routine antibiotics before dental work for most people with a joint replacement. The episode also covers the grey areas where antibiotics may still make sense, and why that decision should be made with your surgeon. Artificial heart valves are a separate case: because they're exposed to blood flow all the time, antibiotics are still recommended before certain dental procedures. If you're scheduled for a hip or knee replacement, timing matters too. The doctors explain why major dental work is best kept at least three months away on either side of surgery, and why an active abscess can't wait. Bottom line: most healthy people with a joint replacement can skip the antibiotics at the dentist, but your own surgeon's advice comes first.
