AI in dermatology struggles with how dermatologists actually think, like the ugly duckling sign for melanoma. Veronica explains why translating whole-patient clinical reasoning into machine learning is so hard. Veronica explains the ugly duckling concept: a lesion that looks very different from the patient's other lesions is often biopsied, though how predictive it is for melanoma is still unknown. She notes clinicians also rely on patient history, three-dimensional cues like whether a lesion is raised, and features such as ulceration that may not show in an image. Her team built a dataset with many lesions per patient, sometimes up to 100 photos, but images taken at different times with different cameras, lighting and distances were flagged as outliers. She calls it a classic case of an algorithm overfitting to something unintended. Veronica describes ongoing work on domain-specific augmentation tools to mitigate camera variability, which she says is challenging.
