Free → https://pilotdebrief.com/pattern On a February afternoon in 2022, a student pilot flying a 60-year-old Piper Cherokee solo out of Savannah heard the engine quit on base leg — with a river, not a runway, ahead of him. The 1961 PA-28-150 had launched with two 24-gallon wing tanks and 4.1 hours already flown since the last fill-up. On paper, that left a thin margin. In practice, no one knew exactly how thin, because the airplane's fuel quantity gauge was broken — and during preflight, neither the student nor the instructor put eyes on the actual fuel level in the tanks. The flight ran longer than a nominal cruise profile suggested. Engine data later showed fuel flow above 10 gallons per hour for 21 minutes of the 1.1-hour sortie, well above the 9 gph book number. After a tower-directed go-around and pattern reentry, the engine quit on base. The student judged he couldn't make the runway and put the Cherokee into the Savannah River. The airplane was substantially damaged. He walked away. Investigators found the fuel selector on the right tank, with about one gallon remaining in it — and two to three gallons sitting unused in the left. No mechanical anomalies, no fuel spillage at the crash site. The NTSB determined the probable cause was the student pilot's improper fuel management, resulting in a total loss of engine power due to fuel starvation. Fuel was on board. It just wasn't the tank the engine was drinking from. The pattern here is older than the airplane: a known-broken gauge, a skipped visual check, an assumed endurance number, and a fuel selector left where it sat. Each link is small.
