In this new video of our educational series, where we examine commonly used basic suturing techniques in surgical practice step by step with the narration of Prof. Dr. Bülent Saçak, we take a practical look at the horizontal mattress suturing technique. The horizontal mattress suture is one of the important techniques that stands out by providing a wide tissue purchase area, distributing tension over a wider surface, and creating mild wound edge eversion. Since it provides temporary strong approximation, it may be preferred in cases where wound edges need to be brought together in a controlled manner. This technique can be used to support wound edge eversion, especially in areas prone to inversion. It offers an effective option for closure in areas with a risk of inversion, such as the palm, and for aligning asymmetric wound edges before skin suturing. The horizontal mattress suturing technique can be used on the palm, palmar surface of the hand, sole of the foot, and areas with a high risk of inversion. Non-absorbable monofilament polypropylene or polyamide suture materials are generally preferred. Suture sizes of 3/0–4/0 may be used on the palm and palmar surface of the hand, 2/0–4/0 on the sole of the foot, and 3/0–4/0 in areas with a high risk of inversion. In the horizontal mattress suturing technique, maintaining equal bite depth and width is important for balanced approximation of the wound edges. Excessive knot tension may impair tissue circulation and lead to a risk of ischemia. If needed, a bolster may be used to reduce pressure. In this application, secure closure is achieved with one full knot and two half-hitches.
