In the ninth video of this educational series, where Prof. Dr. Bülent Saçak presents fundamental suturing techniques used in surgical practice step by step, we examine the half-buried subcuticular suturing technique through a practical demonstration. The half-buried subcuticular suturing technique is used at the junction of thin and thick tissue, as well as in the closure of special border areas such as skin grafts and the areola. Its ability to leave no visible marks on the thicker side while maintaining eversion on the thinner side is among the key advantages of the technique. This technique is applied to approximate tissue edges with different thicknesses and to provide controlled closure in border areas where eversion is critical. The areola-skin border, thin flap-thick tissue junctions and skin graft edges are among the most commonly used areas for this technique. Fast-absorbing multifilament PGLA sutures may be preferred for the half-buried subcuticular suturing technique. Sizes 3/0–4/0 may be used for the areola-skin border and thin flap-thick tissue junctions, while sizes 4/0–5/0 may be used for skin graft edges. During application, the entry and exit should be deep on the thinner side and superficial on the thicker side. This approach helps approximate tissues of different thicknesses in a more controlled manner.
