3 TRT Clinic Mistakes That Could Be Ruining Your Testosterone Therapy Are you on testosterone replacement therapy (TRT) but still struggling with low energy, hormone fluctuations, high estrogen, or unwanted side effects? The problem might not be your testosterone levels. It could be how your TRT clinic is managing your treatment. As a fellowship-trained men's health specialist and urologist, I've seen the same three mistakes repeatedly made by cookie-cutter testosterone clinics. And if you're currently on TRT, there's a chance you're experiencing one of them. Here are the 3 biggest TRT mistakes every man should know: 1ļøā£ Using the Same Testosterone Dosage for Everyone One cc of testosterone cypionate once a week, paired with an estrogen blocker like Arimidex (anastrozole), is a common cookie-cutter TRT protocol. But testosterone replacement therapy isn't one-size-fits-all. Excessive testosterone dosing can increase estradiol (estrogen), elevate hematocrit and red blood cell counts, and contribute to noticeable fluctuations in how you feel throughout the week. For some patients, smaller, more frequent testosterone injections may help maintain steadier hormone levels. Proper TRT dosage, injection frequency, and bloodwork monitoring should be individualized. 2ļøā£ Ignoring the Root Cause of Low Testosterone Before starting testosterone replacement therapy, it's important to understand WHY your testosterone is low. Conditions like obstructive sleep apnea and obesity can contribute to low testosterone levels. TRT doesn't treat sleep apnea, and testosterone therapy may temporarily worsen breathing disturbances in some men with severe sleep apnea. Research also shows that significant weight loss can naturally increase testosterone levels in men with obesity-related low testosterone. Treating the underlying problem can be just as important as optimizing your TRT protocol. 3ļøā£ Failing to Discuss TRT and Male Fertility One of the most overlooked side effects of testosterone replacement therapy is its impact on sperm production. Exogenous testosterone suppresses the hormonal signals responsible for sperm production and can cause severely reduced sperm counts or even azoospermia (zero sperm). In male hormonal contraception studies, approximately 65% of men developed azoospermia within six months of weekly testosterone injections. For men who want children, fertility-preserving strategies involving medications such as hCG, FSH, or alternatives like enclomiphene may be worth discussing with a qualified specialist. These treatments aren't appropriate for everyone, and hCG doesn't guarantee fertility preservation. The bottom line: Effective testosterone replacement therapy involves more than getting your testosterone levels into a target range. The right TRT protocol should consider testosterone dosage, injection frequency, estradiol, hematocrit, sleep apnea, metabolic health, fertility goals, symptoms, and regular bloodwork. If you're already on TRT or considering starting testosterone therapy, make sure your clinic is treating YOU, not just your lab results. š¬ Are you currently on testosterone replacement therapy?
