#정형외과 #골절수술 #골절재수술 #불유합 #불유합수술 안녕하세요. 정형외과 전문의 · 외상학 세부 전문의 김한주입니다. 오늘은 교통사고 후 발생한 고령의 상완골 근위부 골절 불유합 케이스를 소개해드리려고 합니다. 70대 남성분으로 교통사고로 상완골 근위부에 골절을 입으셨습니다. 수상 직후 CT를 보시면 Metaphyseal bone defect(골간단부 골 결손)이 있었고 Medial calcar support (내측 기둥 부위 지지력)이 소실된 상태였습니다. 타원에서 수술을 받으셨는데요, 6개월 후 불유합과 나사못의 밀려나옴으로 저희 병원에 내원하셨습니다. 골결손 부위에 칼슘제제 인공뼈가 들어있었고 뼈는 전혀 붙지 않은 상황이라 변연절제, 금속판 교체, BMP 1mg + DBM 3cc + autobone (자가골 무릎)을 믹스하여 이식해주었습니다.
