In this new video of our educational series, where we examine commonly used basic suturing techniques in surgical practice step by step with the narration of Prof. Dr. Bülent Saçak, we take a practical look at the vertical mattress suture technique. The vertical mattress suture is one of the important techniques used especially for wound closures under high tension. It stands out by providing strong wound edge eversion. Since it helps approximate the wound edges in an everted position, it may be preferred in clinical situations where wound edge eversion is particularly required. This technique can be used in areas where tension may be more prominent, such as the back, trunk, areas over joints, and natural skin folds. Non-absorbable monofilament suture materials such as polypropylene or polyamide are generally preferred. For the back and trunk, 3/0–4/0 sutures may be used; for areas over joints and natural skin folds, 3/0–4/0 sutures may also be preferred. In the vertical mattress suture technique, correct adjustment of knot tension is highly important. Excessively tight knots may impair tissue circulation and lead to a risk of ischemia. Placing the knots at a 90° angle to the incision line is one of the key points to consider for proper application of the technique. In this application, a secure closure is achieved with one full knot and two half-hitches.
