Femoral Broaching Explained: How Surgeons Prepare the Femur for Hip Replacement #shorts . Every cementless hip replacement depends on a step most patients never hear about: femoral broaching. After the femoral neck is cut, the upper thighbone is not ready to accept an implant. The canal is filled with soft cancellous bone and shaped by the patient's anatomy, not by the stem. A broach is a toothed, implant-shaped rasp that carves and compacts that bone into a precise cavity matching the final component. Why it matters: a tight press-fit gives the stem immediate stability and allows bone to grow onto its surface. An undersized stem can micromove. An oversized one can crack the femur. How it is done in the operating room: I open the canal and begin with a broach smaller than my preoperative template. Each broach is seated with controlled mallet blows, then removed, and the next size follows. I keep the broach against the greater trochanter to avoid varus alignment and match the version of the native femoral neck. When the broach stops advancing, the pitch of the mallet changes, and it resists rotation, I know the size is right. The final broach then becomes my trial. With a trial neck and head attached, I reduce the hip and check leg length, offset, range of motion and stability before the real implant is ever opened. For surgeons: respect hoop stress. If a broach meets unexpected resistance, step down a size rather than risk an intraoperative fracture, and use caution in narrow champagne flute (Dorr A) canals, where distal engagement can trap the broach.
