What happens when the body can’t stop bleeding? This is the story of a 13-year-old boy with hemophilia, and the biology behind it. In May 2024, he was admitted to a hospital in Nagpur with severe abdominal pain and breathing difficulty. An ultrasound found about 400 ml of blood in his abdomen. He had been diagnosed with hemophilia at the age of five. To see what went wrong, start with a normal cut. Platelets rush to the injury, and clotting factors in the blood trigger a chain reaction that ends in a clot. In hemophilia, one factor in that chain is missing or deficient. In hemophilia A it is factor VIII. The problem isn’t that the body bleeds more, it’s that the body can’t properly stop the bleeding. Hemophilia A is an inherited, X-linked recessive disorder. The gene for factor VIII is on the X chromosome. A boy has one X and one Y and gets his X from his mother. If that X carries a hemophilia-causing variant, he has no second X to compensate, which is why the condition appears mostly in males. About 1 in 10,000 people is born with hemophilia. Treatment centres on replacing the missing factor. According to the World Federation of Hemophilia, each unit of factor VIII per kilogram of body weight raises the blood level by about 2%. In this case, the boy received 2,000 units of Factor VIII, donated through a haemophilia society, and 1,000 more the next day. He spent five days in the ICU, a CT scan two days later showed the bleeding had stopped and the collected blood was reducing, and he was discharged after ten days.
