The brakes had failed on four previous flights. Each time, a pilot used the emergency braking system to stop the airplane. Then two passengers boarded—and a different crew faced the same failure. On September 27, 2018, a Dassault Falcon 50 landed at Greenville Downtown Airport in South Carolina. The thrust reverser deployed, but the jet kept going. Seconds later, the cockpit recording captured the question: “Where are the brakes?” The airplane overran the runway and plunged down an embankment. Both pilots suffered fatal injuries. Passengers Marci Wilhelm and Steve Rose survived with serious injuries. Using the NTSB investigation, cockpit transcript, maintenance records, and testimony from the subsequent civil litigation, we examine how this flight came together—and why several opportunities to prevent the crash were missed: • Why the brakes could work during taxi but fail at landing speed • The crew’s qualifications for their respective roles • Four earlier flights that exposed the braking problem • Unfinished maintenance, unclear documentation, and a critical brake-selector setting • The emergency braking options available to the crew I also explain where I believe the investigation left important safety questions unanswered, distinguishing the documented findings from my analysis. **Sources and supporting documents:** [NTSB final report — ERA18FA264](https://data.ntsb.gov/carol-repgen/api/Aviation/ReportMain/GenerateNewestReport/98361/pdf) [NTSB investigation docket](https://data.ntsb.gov/Docket?ProjectID=98361) Falcon 50 pilots and maintenance technicians: if you have relevant experience with the antiskid system, brake-selector procedures, or applicable MEL provisions, share it below.
